Lead Practitioner in Adult Care – Level 4 Apprenticeship

The Lead Practitioner in Adult Care will guide and inspire team members to make positive differences to someone’s life when they are faced with emotional physical and other challenges. They will have achieved a level of self-development to be recognised as a leading and guiding mentor within a care team, ‘that go-to person’ with a greater depth of knowledge and expertise in their area of responsibilities which allows them to lead in areas such as care needs assessment, assistive technology, rehabilitation and enablement. They will also have a role in the assessing of others performance and the quality of care delivery and coach and mentor staff.

  • You will be allocated a dedicated Teaching Assessor.
  • An Individual Learning Plan will be developed with you and your Teaching Assessor.
  • You will have formal progress reviews with your Assessor and Manager every 8-10 weeks.
  • Coaching & Mentoring forms an integral part of your individual learning journey to ensure you have the most appropriate guidance and support to complete your programme.
  • At the end of your programme you will complete an End-Point Assessment with an independent Assessment organisation, agreed by your employer.
  • You will mutually agree SMART targets that take into consideration your individual ambitions and career goals.

Suitability - Who should attend?

Who it's for:

The Lead Practitioner in Adult Care will guide and inspire team members by coaching and mentoring staff to obtain high quality care delivery and have a depth of knowledge and expertise in their area of responsibilities which allows them to lead in areas such as care needs assessment, assistive technology, rehabilitation and enablement. They can work in residental homes, nursing homes, domicillary care settings, persons own home and some clinical health care settings.

The course is made up of several elements :

  • Level 4 Standard in Lead Practitioner in Care
  • Level 4 Diploma in Adult Care
  • Level 2 English and maths functional skills

Undertake the Disclosure and Barring service process and provide the result.
