The Safety and Accident Investigation Centre at Cranfield University has partnered withBaby Lifelineand Baby Lifeline Training to develop a practical, skills-based training course specifically in healthcare incident investigation.
This course covers the skills required to carry out an effective healthcare based incident investigation. Subjects covered include interviewing, analysis, family engagement, human factors and report writing.
The way healthcare investigates and learns from patient safety incidents is increasingly in the public eye and as such the need for a professional approach to investigation is essential at all levels. This need has been highlighted in numerous healthcare reports published over the last 5 years. Cranfield Safety and Accident Investigation Centre has over 40 years experience training safety investigators. We are now pleased to build on a recent history training national healthcare investigators and expand this knowledge exchange to a Trust level cohort.
This course blends current healthcare investigative experience with a body of investigative academic knowledge built up over many years. Investigative theory is delivered during the week and put into practice in a concurrently running simulated scenario. This scenario evolves during the week with delegates requesting information, calling forward interviewees and developing analysis models based on the evidence collected. As the delegates investigative skills grow throughout the week they employ these skills to unravel the incident in question and develop recommendations to prevent reoccurance.

Suitability - Who should attend?
The course is of benefit to those healthcare professionals who are required to either carry out or review safety investigations. The simulation will be particularly relevant to the maternity setting, but the principles, techniques, skills and methodology taught is equally applicable to all types of healthcare incident investigation.

What you will learn
The course will build up the delegates’ professional knowledge base to equip them with additional investigative skills. On completion of the course, delegates will be able to:

- Establish and record a sequence of events.
- Identify and record causal and contributory factors relating to the case.
- Engage and involve families.
- Gain interview techniques (structured, semi-structured, cognitive).
- Develop and write appropriate reports and healthcare recommendations.
- Deliver and understand regulatory and NHS patient safety policies.
- Understand systemic investigations and human performance issues.
- Understand organisational factors and safety culture.
Course delivery details
Course structure
This five-day course is presented online through a mixture of live and recorded lectures, team investigation activity, team presentations, tutorials and discussion of presented case studies. Active participation from delegates throughout is strongly encouraged, this is particularly important in the simulation phases to consolidate learning. All delegates will receive a Continual Professional Development Certificate upon completion of this course.
